Managing SaaS Price Increases at Contract Renewal

Hi Everyone,
Our team working to decide on a framework to increase our MRR pricing for customers upon contract renewal (SaaS). How have others thought through this exercise?
1. Do you treat all customer segments considered equal?
2. Does pricing increase based on a set $ amount ($100/month increase) or % based on contract value (20% of MMR)?
3. How and when do you communicate this to customers? Is it passive or pro-active conversation and how do you manage this conversation with customers that are a high churn risk?
